Hypoglycemic drugs
Tirzepatide is a new type of glucagon-like peptide-1 receptor (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP, also known as gastric inhibitory polypeptide) dual receptor agonist, which is administered once a week. Both GLP-1 and GIP are incretins, which are polypeptides secreted by the gastrointestinal mucosa of the human body. The former can bind to receptors on pancreatic islet cells and stimulate the secretion of pancreatic hormones, thereby producing a hypoglycemic effect, and can also delay gastric emptying and suppress appetite, thereby controlling weight; while the latter has the functions of inhibiting gastric acid and pepsin secretion, stimulating insulin release, inhibiting gastric motility and emptying, etc., which can supplement the effect of GLP-1 receptor agonists. Tirzepatide integrates the effects of two insulinotropic agents into a single molecule, representing a new class of drugs for the treatment of type 2 diabetes.
Uses
Tirzepatide has a similar sequence to semaglutide. The lysine side chain in the sequence is modified with PEG, which is a functional group of the peptide and can increase the water solubility of the sequence. The main physiological function of Tirzep Chemicalbookatide is a dual agonist of glucose-dependent insulin polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors. It is being developed for the treatment of type 2 diabetes and has entered the clinical stage.
Safety
The safety of telopoietin is similar to that of other incretin drugs. A meta-analysis of 7 randomized controlled trials showed that the gastrointestinal reactions (such as nausea, diarrhea, and constipation) of telopoietin were significantly improved compared with placebo, but most of them were mild to moderate. No significant differences were found between the two groups in other adverse reactions, especially hypoglycemia. However, due to its short time on the market, its actual safety still needs to be demonstrated after it is launched on the market.
Effects of hypoglycemic and weight loss
The SURPASS study is a series of large-scale randomized controlled clinical trials designed to evaluate the efficacy and safety of tirpotide in patients with type 2 diabetes (T2DM). Among them, the results of SURPASS-2[1], which was a head-to-head comparison with semaglutide, were the most exciting. The study compared the hypoglycemic effects of tirpotide 5mg (n=470), 10mg (n=469) and 15mg (n=469) with semaglutide 1mg (n=468). The results showed that compared with the baseline HbA1c level (8.3%), tirpotide reduced the subjects' HbA1c by an average of 2.0%, 2.2% and 2.3%, while semaglutide reduced it by an average of 1.9%; in terms of weight loss, compared with the Chemicalbook baseline weight (207 pounds), tirpotide reduced the subjects' average weight by 17 pounds, 21 pounds and 25 pounds, while semaglutide reduced it by 13 pounds. The trial showed that tepoxetine showed better blood sugar lowering and weight loss capabilities than semaglutide. Another large study recruited 2,539 adults with at least one obesity complication (excluding diabetes) with a body mass index (BMI) ≥30kg/m2 or ≥27kg/m2. All subjects were randomly divided into tepoxetine 5mg, 10mg, 15mg and placebo groups in equal proportions. The results showed that at 72 weeks, the weight of each group decreased by 16.1kg, 22.2kg, 23.6kg, and 2.4kg, respectively[2]. The weight loss effect of tepoxetine is comparable to that of bariatric surgery.
